Your pulmonary function test results are the most important factor. Underwriters focus on your FEV1 (how much air you can forcefully exhale in one second) and your FVC (total lung capacity). These numbers tell them how much your breathing is actually affected day to day.

Whether you’re currently using oxygen is a critical dividing line. Routine supplemental oxygen use is typically an automatic decline for traditional life insurance across most carriers. If you need oxygen, guaranteed issue coverage may be your best path forward.

Your smoking history plays a significant role. COPD caused by smoking generally results in higher rates or more restrictive underwriting than COPD from other causes like occupational exposure or alpha-1 antitrypsin deficiency. If you’ve quit smoking, how long ago you quit matters too.

Hospitalizations and ER visits in recent years signal severity to underwriters. A history of frequent exacerbations or hospital stays suggests the condition is less stable, which affects how carriers view the risk.

Your medications tell a story about severity. Someone managing COPD with a single daily inhaler looks very different to an underwriter than someone on multiple bronchodilators, oral steroids, and nebulizer treatments.

How Severity Affects Your Options

Mild COPD

If your FEV1 is above 65-70% of predicted and you’re managing symptoms with one or two inhalers, many carriers will consider you for traditional term or whole life coverage. You’ll likely pay more than someone without COPD, but the rates can still be very reasonable, especially if you’re a non-smoker with no recent hospitalizations.

Some carriers are more flexible with respiratory conditions than others, so it’s worth comparing offers rather than accepting the first quote you receive.

Moderate COPD

With FEV1 between roughly 50-65% of predicted, your options narrow but don’t disappear. Several carriers will still offer traditional coverage, though at higher rates. The key factors at this stage are stability (no recent hospitalizations or worsening symptoms) and compliance with your treatment plan.

Simplified issue policies that skip the medical exam can also be a good fit here. These policies use health questions rather than lab work, and some are more forgiving of moderate COPD than traditional underwriting.

Severe COPD

If your FEV1 is below 40%, you’re on supplemental oxygen, or you’ve had multiple hospitalizations, traditional underwriting will likely result in a decline. But that doesn’t mean you’re without options.

Guaranteed issue life insurance is designed specifically for situations like this. You can’t be turned down regardless of your health, and you won’t answer any medical questions. The tradeoff is lower coverage amounts (typically up to $25,000-$50,000) and a graded benefit period during the first two to three years. After that waiting period, the full death benefit is in effect.

For someone whose primary concern is covering funeral costs and final expenses, guaranteed issue can provide genuine peace of mind.

COPD rarely exists in isolation. Many people with COPD also have high blood pressure, heart disease, or diabetes, and underwriters consider the full picture. Having one well-managed condition alongside mild COPD is very different from having multiple uncontrolled conditions.

If you have COPD plus other health concerns, that’s actually a reason to work with someone who knows which carriers weigh those combinations more favorably. The difference between a carrier that treats COPD harshly and one that takes a more nuanced view can be significant.

Steps You Can Take Before Applying

You can’t change your diagnosis, but you can put your best foot forward during underwriting.

Get a recent pulmonary function test. If your last PFT is more than a year old, ask your doctor for an updated one. Underwriters want current numbers, and if your condition has been stable or improving, fresh results work in your favor.

Document your treatment compliance. Regular doctor visits, consistent medication use, and following your treatment plan all signal to underwriters that you’re managing the condition responsibly.

If you smoke, quit. This is the single most impactful thing you can do for both your health and your insurance options. Most carriers want to see at least 12 months of being smoke-free before they’ll consider non-smoker rates, though some require longer.

Gather your medical records. Having your records organized before you apply speeds up the process and reduces the chance of delays or miscommunication between your doctor’s office and the insurance company.
